Staff members with the Utah Department of Workforce Services will help seniors to apply for heating assistance through the HEAT (Home Energy Assistance Target) program.  They will be at:

Ґ North Davis Senior Activity Center, Nov. 21, 9 a.m.- 4:30 p.m. Call 801-525-5080 for appointment.

Ґ Autumn Glow Senior Activity Center, Dec. 4, 9a.m. - noon. Call 801-544-1235 for appointment. 

Ґ Golden Years Senior Activity Center, Dec. 4, 1 - 5p.m. Call 801-295-3479 for appointment.

 Bring all the necessary documentation (must be presented before the application will be started) including:

Р Identification: Current driver’s license or state ID. 

Р Social Security cards for yourself and all persons in your household.

Р Proof of your most recent and active utility bills from each utility supplier (i.e., both gas and electric) or/and a landlord statement.

Р Proof of all gross income (earned or unearned) received by all household members in November 2013. If no income exists, please complete a written statement explaining how expenses are paid and why no income for the month of November 2013. Each adult must complete one if there is no income; please include date and signature.

Р Proof of medical expenses you paid out in November 2013.

Р Proof of any child support and/or alimony you paid in November 2013.

Ґ Proof of disability (must be at least 6 months).

Ґ Proof of age of a child in the home age (5) and under (birth certificate, blessing certificate, etc.).

 Additional documentation may be required.  Eligibility requirements for HEAT assistance:

The total household income is at or below 150% of the Federal Poverty Level. The household is responsible for paying home energy costs. The household contains at least one U.S. citizen or qualified non-citizen.
